On your first visit, we will ask you for some information about yourself and your insurance carrier if you have one. 

We will want to make a copy of your tribal enrollment card, insurance card (if you have), picture ID and proof of income (if applying for sliding fee scale discount). 

You will also be asked to complete new patient forms including patient information and medical history. You may want to save some time by printing the forms, filling them out and bringing them with you. All forms can be read in Acrobat Reader.
