Chicago Health Clinic & Behavioral Health

Our Medical Clinic features four specialized practice rooms. Within these state-of-the-art facilities, our skilled team of healthcare providers, including doctors, nurses, and support staff, deliver comprehensive medical care through both in-person and telehealth services.

Our Behavioral Health Offices, conveniently situated next to our Medical Clinic, are equipped with five dedicated practice rooms. Here, our team of experienced providers, therapists, and social workers offer comprehensive care both in-person and through telehealth services.

Our facility offers activity and meeting rooms that serve as vital spaces for our gatherings, programs, and meetings. These rooms play an important role in bringing the community together, fostering healing, and strengthening cultural and community ties.

What to Expect: Your First Visit

To ensure everything goes smoothly, please arrive 1 hour before your scheduled appointment for registration. You will need to bring the following documents to complete your registration:
  • Eligibility Documents (CDIB or federally recognized tribal enrollment card, letter of descendancy from a federally recognized tribe)
  • State-issued photo ID (for individuals aged 18 and over)
  • Certified birth certificate (for children under 18)
  • Social Security Card
  • Immunization records for children under 16
  • Medication profile (list of all prescription and over-the-counter medications)
  • Insurance information (if applicable)
