The 340B Program is a federal drug discount program that enables AIHSC to provide NO COST formulary medications to the entire, eligible tribal population in Illinois and surrounding states.
Eligible patients must be an enrolled member of a U.S. federally recognized tribe and possess a tribal ID or Certificate of Indian Blood. Prescriptions must be filled at one of AIHSC’s registered Walgreens 340B pharmacies. The patient must have active health records with AIHSC.

Eligible uninsured and insured patients will be able to get formulary medications at NO COST since formulary medication co-pays will be waived.
Yes, it is possible to keep your current primary care provider & your current specialists. But you must establish care with AIHSC or have an active patient chart.
Co-Pays waived and no cost if uninsured. Medicaid coverage not included.
• NO co-pays for all clinic visits and prescribed formulary medications.
• Obtain AIHSC referrals to specialist providers so these prescribed medications are no cost.
• Get required labs drawn for your primary care provider and specialty providers to ensure proper dosing and utilization of medications.
• Maintain your current health data in AIHSC’s electronic health record to ensure appropriate coordination of care for chronic conditions that are prevalent
among the tribal populations (diabetes, heart disease, hypertension, high cholesterol, etc)
Step 1: Call (773) 883-9100 to establish care or set up an annual visit to AIHSC located at 4326 W. Montrose Ave., Chicago, IL.
Step 2: Provide a copy of your medical records prior to appointment. A Notice of Privacy Practices form will be available when you register.
Step 3: Present your tribal ID and insurance information at your first appointment.
Step 4: Bring AIHSC Provider Voucher(s) when picking up prescriptions at one of our Walgreens 340B Pharmacy Locations.
Step 5: Be sure to follow-up with your AIHSC provider at least annually or as required
