Welcome to The American Indian Health Service of Chicago (AIHSC)!
We’re thrilled to welcome new patients and ensure that you have a positive, supportive experience as you begin your health care journey with us. Our compassionate team is here to provide the personalized care you deserve, whether you are insured, uninsured, or facing financial challenges.
Getting Started
We make it easy for you to begin your healthcare journey at AIHSC with a straightforward registration process. To get started, simply schedule your appointment by calling (773) 883-9100 or emailing us at [email protected].
What to Expect: Your First Visit
To ensure everything goes smoothly, please arrive 1 hour before your scheduled appointment for registration. You will need to bring the following documents to complete your registration:
- Eligibility Documents (CDIB or federally recognized tribal enrollment card, letter of descendancy from a federally recognized tribe)
- State-issued photo ID (for individuals aged 18 and over)
- Certified birth certificate (for children under 18)
- Social Security Card
- Immunization records for children under 16
- Medication profile (list of all prescription and over-the-counter medications)
- Insurance information (if applicable)
Insurance and Payment Options
We accept a variety of insurance plans, including Medicaid, Medicare, and private or tribal insurance. If you have coverage, please provide your insurance card(s) at each visit. If you don’t have insurance or if you’re facing financial difficulties, we offer a sliding fee scale based on your income, so you can still receive the care you need.
